Healthwatch H&F wants to find out from a wide range of people about how well social prescribing services in the borough are working, and where they could be better.
What are social prescribing services?
Social prescribing services provide non-clinical solutions for patients with health and wellbeing issues, such as ill mental health, loneliness, or financial pressures, by providing social, emotional and practical care.
It is part of the holistic, personalised care approach to healthcare that encourages people to have greater choice and control over their lives and addresses the wider determinants of health, while reducing pressures and reliance on primary care professionals and resources. This is done by supporting patients – particularly more marginalised groups - to address the underlying issues affecting their health and wellbeing. Increasingly, social prescribing services are being recognised for reducing health inequalities within communities. Only GP practices can refer patients aged 18+ to a social prescribing link worker.
What are social prescribing link workers?
Social prescribing link workers spend time with the patient to develop their personal plan to improve the patient’s health and wellbeing. In Hammersmith and Fulham, link workers provide up to six, 45-minute sessions, where they will share information with the patient about local social prescribing services that might help them, such as physical activity, cooking, financial support, gardening, arts and crafts classes, or befriending schemes. How will your response make a difference?
Healthwatch Hammersmith and Fulham will be very grateful to everyone who completes this survey, as it will make an important contribution to improving social prescribing services in Hammersmith and Fulham. We will use these responses and additional research to produce a report that will share key findings and make recommendations for improvements to the service.
The survey is open to the public on Wednesday 1st February and closes on Friday 31st March at 12pm.
